Canine Cognitive Dysfunction (CCD) is a degenerative brain disease affecting older dogs. By age twelve, roughly a third of dogs show some signs. It's easy to mistake for "just getting old."
The DISHA framework
Vets use five letters to describe the syndrome. Disorientation — getting stuck behind furniture, staring at walls, going to the wrong side of the door. Interactions — less interest in greetings, less affection, snappier with family members. Sleep cycles — pacing at night, sleeping more in the day, waking confused. House soiling — accidents in a previously house-trained dog. Activity changes — repetitive pacing, slower response to commands, less play.
Confirming the diagnosis
There's no blood test for CCD. The vet rules out other causes first — kidney disease, thyroid problems, vision loss, hearing loss — then makes a clinical diagnosis based on the DISHA signs. Once confirmed, treatment focuses on slowing progression rather than reversing it.
Diet and supplements
Diets rich in medium-chain triglycerides (MCT), antioxidants, and omega-3 fatty acids slow cognitive decline measurably in studies. Prescription cognitive diets exist; coconut oil added to food is a budget alternative under vet guidance. SAMe and phosphatidylserine supplements are sometimes added.
Routine, routine, routine
Dogs with CCD anchor themselves to schedule. Feed, walk, sleep, and toilet at the same times daily. Move furniture less often. Keep food and water bowls in the same place. Predictability lowers anxiety, which lowers night pacing.
Enrichment counters atrophy
Mental work slows cognitive decline. Slow feeders. Snuffle mats. Nose-work games. Teaching one new, easy command per week — even at age fifteen. The brain that gets used loses function more slowly than the brain that rests.
The night-time problem
Disrupted sleep is the symptom that breaks most owners. A nightlight in the room. A snug bed in the corner of the bedroom (anxiety is worse in unfamiliar dark). Discuss melatonin or anxiolytics with your vet for severe cases — they work and they don't shorten lifespan.
